I would like a copy of my background check. How will I receive that?
If you would like a copy of the reports(s) please check the box on the Background Check Disclosures Page. The report(s) will be sent to you at the address listed on the background check form. You may also request a copy of your background check by contacting DISA at 1-800-938-2114.
