Can I view a summary of the health, dental, and vision coverage options?

The benefits guide is now available to provide a summary of benefits at DISA. Additionally, training sessions and one-on-one opportunities are available for anyone with detailed questions about their needs.



Where can I access plan documents and SBCs for each of the medical plans?

These documents are being worked on as of 11/6. They will be available during open enrollment in the Documents and Resources section located above.



Can I keep my current plan? 

Yes, all our current plans will continue in 2025 with just a few changes.



Will we automatically be enrolled in the same plan as last year? 

This year all employees will be required to take action in Workday for their benefits elections. Most plans will be pre-populated with your current elections, but you will need to take time to review and submit your elections. We will cover these requirements during the benefit training sessions.



My doctors have never heard of Imagine 360 (i-360) does that mean I can’t use this insurance? 

The low and high-deductible plans have been specifically designed to allow you to choose from any provider without being connected to a network. As long as the provider accepts our insurance, you can continue to see them. This plan is unique, and we have several ways to work with providers that may be unfamiliar to you and the provider. If you are confused or struggling with a provider, please reach out to the benefits team to assist. We have been able to set up provider relationships 99% of the time, but it can take a few extra steps to ensure a successful connection.



Who is the health insurance provider?

All our medical plans will continue to be administered by Imagine 360 but will have different networks depending on the plan you select and your geographic location.



If I sign up for the new CIGNA Network Plan, how do I know if my doctors are in the network? 

To check if your doctors are in network for the Network plan on Cigna, please visit the following link and select Open Access Network.



If I sign up for the Low or High-Deductible plan, how do I know if I am in an Imagine Health Area? 

Each employee on these plans is designated for either the Imagine Health or the Non-Imagine health plan. This is determined by I-360 based on your geographic location. You can see which network you are in based on your ID card. If you are in the Imagine Health network, you will see an Imagine Health Logo on the front of the card.



Does the company pay for or offer reduced premiums for employees?

Yes, DISA covers on average 70+% of benefits coverage regardless of your chosen plan. We believe affordable healthcare is something all our employees should have.



How do I select my benefits? 

Open enrollment runs from midday November 11th through November 20th, during which you can log into Workday to enroll in your 2025 coverage.



Are we going to have any changes to our 401k plan?

There are no changes to the DISA 401(k) plan other than the annual updates on maximum contributions allowed. As a reminder you can change your 401K elections as often as you want at www.401k.com



Are we going to have any changes made to our life insurance plan?  

We are making changes to our life insurance and ancillary plans. You can find detailed information about these plans in the benefits guide and when talking to ALEX our new AI generated benefits assistant.



I currently call Quantum for all my questions, but I know we are removing them from the process in 2025. Can I get the number for I-360 to call now?

We will continue to have employees and providers work directly with Quantum until the end of 2024. Information will be coming out before the end of the year for I-360 who will be our contact for medical insurance in 2025.

Can I waive my benefits for a spouse's plan or the exchange?  

Yes, at open enrollment you can choose to waive your benefit coverage from DISA. If choosing to go on the exchange instead, we recommend you consult a tax advisor so that you fully understand the tax implications. You will not be eligible for any of the subsidies because our plans meet the requirements for coverage. We will ask for proof of coverage if making this decision. You will also have an opportunity to change this election at the next enrollment period or if you experience a qualified life event.



Is the network change for pharmacy benefits? How do I know where I go for prescription questions?  

There are no changes for our pharmacy benefits and all plans will continue to utilize RX Benefits and Express Scripts for our pharmacy needs. The contact for RX Benefits is 1-800-334-8134 or Member.RXBenefits.com.



Will I receive new medical ID cards?  

All employees will receive new ID cards for 2025. Our goal is to have those to employees by the end of December. You will also be able to access cards digitally in December through the I-360 portal.
